Clicking on the sauna, both sims have to choose Enjoy Refreshing Steam.
Once seated, have one sim click on the other and choose Cuddle.
When cuddled up, click on the other sim again (not the sauna) and choose Sauna Woohoo.
Then make the choice between woohoo or try for a girl or boy baby. (My baby options are greyed out here because...well, ahem. Anyway.)
A sim will always get up to refresh the steam when changing an option, while the other sim waits.
Woohoo takes place behind a cloud of steam.
And that's it!
